While enrolled in externship:
Learning Agenda (PDF) » Due 2 weeks after start of externship or by January 27, 2010, whichever is earlier.
Goals and Experiences Worksheet for Learning Agenda (PDF) » For student use.
Extern’s Mid-Semester Conference Report (PDF) » Student should bring this form to their Externship Conference with the Externship Director. Information about conference dates and times will be sent to all externs after the start of the semester.
Externship Project List (XLS) » Student should bring this form to their Externship Conference with the Externship Director. Information about conference dates and times will be sent to all externs after the start of the semester.
Weekly Timsheet/Journal » Every Monday by 11:59 p.m.
